Medicaid Managed Care Provider Addenda

Managed Care Entities (MCEs) are required to enter into fully executed subcontracts with their providers, unless specified in the managed care provider agreement or Ohio Administrative Code. Subcontracts must include a baseline contractual agreement, as well as the appropriate model Medicaid Addendum. The model Medicaid Addenda incorporate all applicable OAC rule requirements specific to provider subcontracting, therefore, modification is strictly prohibited, except for adding personalized information such as the MCE's name.

Medicaid Addenda are used when contracting with providers for the provision of health care services to members.
