Waiver Services

Medicaid home and community-based waivers allow individuals with disabilities and chronic conditions to receive care in their homes rather than in long-term care facilities, hospitals, or intermediate care facilities. More Information

HOME Choice

Helping Ohioans Move, Expanding Choice (HOME Choice) is a transition program that assists persons, age 18 and older, with any type of disability move from a long-term care facility (like a nursing facility or hospital) into a home and community-based setting. More Information
