Public Notice and Request for Comment
Pursuant to the provision of title 42 Sections 441.301 and 441.304 of the Code of Federal Regulations, public notices are required for any of the following: new 1915(c) waiver, new 1915(i) state plan amendment, renewal of a 1915(c) waiver, and any amendment to a 1915(c) waiver that includes one or more substantive changes.

| Purpose |
The Self-Empowered Life Funding (SELF) waiver amendment proposed for adoption on July 1, 2022, is a result of several years of stakeholder engagement between the Department of Developmental Disabilities (DODD), the Ohio Department of Medicaid (ODM), families and individuals, advocates, and associations. This work has culminated in changes to the operationalize of self-direction in Ohio’s developmental disability system, service delivery changes, service additions, and rate enhancements in the SELF waiver. |

As part of the waiver amendment process, the public can comment on the proposed amendment. The public comment period is February 8, 2022 - March 10, 2022 Comments must be received by midnight on March 10, 2022, and can be submitted by:
