Targeted Case Management

Targeted case management services assist individuals with intellectual disabilities gain access to needed medical, social, and educational services. The Medicaid-provided service includes assessment, care planning, referral and linkage, monitoring and follow-up.

Groups eligible for Targeted Case Management include:
  • individuals eligible for Medicaid, regardless of age, who are enrolled on a home and community-based services waiver;
  • individuals eligible for Medicaid, age 3 or older, who have been determined to have an intellectual disability; and
  • individuals eligible for Medicaid and transitioning to a community setting.

In Ohio, only a County Board of Developmental Disabilities can provide targeted case management services to individuals with intellectual disabilities.
