Ohio Access Success Project

The Ohio Access Success Project (Success Project) is a statewide program that provides eligible nursing facility residents with one-time funding to assist with relocation expenses.

To be eligible for the Success Project, a person must be age 20 or over, and
  • be receiving Medicaid-funded nursing facility services at the time of the application;
  • be able to remain in the community as a result of having received project benefits;
  • not qualify for similar services that are reimbursable by Medicaid; and
  • have been determined to be INELIGIBLE for the HOME Choice program (i.e., they will not be in a nursing facility for 90 days, they are not moving into a qualified residence, they are moving out of state, etc.).

To apply, complete the Ohio Access Success Project Applications (ODM 02372 form).
